By filling out this form you agree to the following: Being the parent or legal guardian of the minor(s) listed below, have been informed of the activities sponsored by Fletcher First Baptist Church AWANA ministry and hereby give my consent for my minor child(ren) to participate in these activities. I understand that all reasonable safety precautions will be taken by the leaders of these activities and that the possibililty of an unforeseen hazard does exist. I consent to emergency medical treatment for my child(ren). I understand that all efforts will be made to reach me prior to treatment using the phone numbers I provide below. In the event that I cannot be reached, I giver permission to the Commander/Pastor to make decisions necessary for treatment. I am responsible for the health care decisions for my child(ren) and agree that my insurance plan is the primary plan to pay for their medical treatment. Fletcher First Baptist Church AWANA offers Puggles, Cubbies, Sparks, Truth & Training, Trek and Journey. Awana covers Birth to High School Seniors. We sometimes take pictures of the children during their activities for use in newsletters, promotional materials, bulletin boards and the church website. By signing this form, you are giving us permission to use your child's photograph solely for these purposes. (For safety reasons, their names will not be printed with their pictures.)

Our Ministry's Purpose
Our Ministry's Purpose: To reach boys and girls with the Gospel of Christ and train them to serve Him. Our Awana ministry exists to teach children the Word of God and help lead them to Christ. We also serve them in their growth as Christians in their daily lives. Awana Rules Children come to Awana to learn about God, His Word, and connect with each other. Behavior that hinders this goal, individually or collectively, will not be accpeted. Therefore, at Awana events, all youth will be expected to... • Respect and obey staff. Hebrews 13:17 -- "Obey them that have the rule over you, and submit yourselves: for they watch for your souls, as they that must give account, that they may do it with joy, and not with grief: for that is unprofitable for you." • Stay with the group at all times. Ephesians 4:3 -- "Endeavoring to keep the unity of the Spirit in the bond of peace." • Speak and act with love and encouragement. Ephesians 4:29 -- "Let no corrupt communication proceed out of your mouth, but that which is good to the use of edifying, that it may minister grace unto the hearers." • Keep electronic devices turned off or left at home. They are only to be used for emergencies. Philippians 2:3 -- "Let nothing be done through strife or vainglory; but in lowliness of mind let each esteem other better than themselves." • Show appropriate public displays of affection. 1 Timothy 4:12 -- "Let no man despise thy youth; but be thou an example of the believers, in word, in conversation, in charity, in spirit, in faith, in purity." • Dress appropriately. 2 Corinthians 6:3 -- "Giving no offence in any thing, that the ministry be not blamed:" Consequences for Disobedience At Fletcher First Baptist Church, it is our goal to handle problems immediately, consistently, and in the spirit of love and gentleness. Using scripture, we will show students how their behavior is not pleasing to God and is harmful to themselves or the other children. The staff will always seek a result that includes confession, forgiveness, and restoration.
